What Is Consumer Directed Services?
Consumer Directed Services (CDS) is a care option that allows individuals to take an active role in directing their own in-home support. This program is designed for people who prefer greater choice and flexibility in how their care is provided.
How CDS Works
With CDS, the individual receiving services directs their own care and selects a personal care attendant they know and trust, such as a friend or family member, this cannot be a spouse. The consumer oversees day-to-day care decisions, while Applewood In-Home Health provides administrative support to ensure program requirements are met.
Care May Include Assistance With:

Dressing and bathing

Toileting and mobility support

Meal preparation and light housekeeping

Transportation related to daily needs and errands

Other routine activities that support independent living
Your Role as the Consumer
The consumer (or their authorized representative) is responsible for choosing and directing their attendant, including scheduling and task direction. Applewood assists with payroll processing, documentation, and compliance requirements so consumers can focus on their care.
Support From Applewood In-Home Health
Applewood serves as the program vendor, offering guidance, resources, and ongoing support throughout participation in CDS.

👥Selected as a CDS Attendant?
If you have been chosen by a CDS consumer to provide care as their trusted caregiver, you may apply to become a CDS attendant.
CDS attendants are typically friends or family members (not a spouse) selected by the comsumer and must complete required onboarding and state documentation before services begin.
